Agreement

In connection with, and in consideration of, the Student's participation in the Program, I, on behalf of the Student and myself hereby represent and agree as follows: (1) I certify that I am the parent or legal guardian of the Student and that I have the right to make decisions for the Student that affect his/her wellbeing. I understand the Student will be a participant in the Program, and I hereby give permission for the Student to participate in the Program.; (2) I grant permission for photographic or audio recording of the Student’s group participation in activities sponsored by Engineering Tomorrow for promotional and/or educational purposes. All information collected on this form is only used to track participation in the Program, improve future programming, and to monitor the success of Engineering Tomorrow's goals (i.e., track how many students study engineering in college and graduate with engineering degrees using the National Student Clearinghouse). Engineering Tomorrow will not sell any of the data that it collects on this form to any third party for any commercial purposes, including for targeted advertising to students.
